Additional Motor Vehicle Form Information
This document is to be signed either by the seller alone, or by both seller and purchaser. It describes the vehicle sold or evenly traded (year, make, and vehicle identification number), the date of the sale or trade and the purchase (selling) price or trade value.
General Affidavit Form (DOR-768)
This document combines six common affidavits in one form. Following are the six different affidavit types. Note that some require notarization of the applicant's signature.
- Non-Use (notary required): Must be completed when applicant asks to license a vehicle that has not been operated on the highways, and applicant wishes to have registration fee prorated. Submitting an affidavit of non-use does NOT exempt the applicant from applicable renewal penalties.
- Gift: Must be completed when a vehicle is given to another individual as a gift.
- Vehicle Out-of-State: Must be completed and signed by the owner of the vehicle when he/she is renewing the license plate while the vehicle is out-of-state (and therefore, has not been safety-inspected). In addition, the applicant must state that the vehicle has not been in Missouri during the 60 days preceding the application.
- Abandoned Vehicle on Real Property: Must be completed when a vehicle was abandoned on real estate that you own or have purchased.
- Repossession Certification (notary required): Must be completed when repossessing a boat or vessel or outboard motor and using the “Application for Missouri Boat/Vessel or Outboard Motor Title and Registration” (Form DOR-93). Written consent must be obtained by all owners and/or lienholders on record or notice must be given to them.
- Other: May be completed by the applicant when a transaction requires any type of affidavit. In these cases, the form is used to outline the particular circumstances surrounding the transaction.
